Anna Walker, MSN, APRN, FNP, is a dual board-certified Family Nurse Practitioner with over a decade of experience dedicated to patient-centered care, nursing education, and workforce development. She is the Founder and President of Family First Nurse Practitioner, a membership-based direct primary care clinic that provides affordable, compassionate healthcare services tailored to the needs of individuals and families in California. With a deep commitment to bridging gaps in the healthcare system, Anna’s mission is to make high-quality care accessible to underserved and uninsured communities, while fostering meaningful patient-provider relationships.

Anna holds a Master of Science in Nursing from West Coast University and has extensive experience in diverse roles across hospice care, case management, and direct patient care. She currently serves as the Director of Patient Care Services at LifeBridge Hospice Care, where she oversees patient care operations, ensures quality standards, and supports both patients and their families through compassionate end-of-life care. She also continues to mentor future nursing professionals as a preceptor at West Coast University and a member of the Occupational Advisory Committee at CES College. Fluent in English, Russian, and Ukrainian, Anna’s multilingual abilities enable her to serve a wide range of diverse populations, addressing the unique cultural and healthcare needs of her patients.

This model allows patients to access primary care services directly from their provider without insurance involvement. In exchange for a membership fee, Family First offers:

  • Up to 15 visits per year (additional visits cost $75 each).
  • Unused visits can roll over to the following month.
  • An annual wellness visit free of charge.

Membership Costs:

  • Enrollment Fee: $49 (individual), $149 (family).
  • Monthly Rates:
    • Youth (12-18): $69/month.
    • Adults (19-62): $115/month.
    • Seniors (65+): $125/month.

For families, rates vary based on size.

Flexible Options:
If membership isn’t suitable, patients can schedule visits individually. Both insured and Medicare patients are welcome.

Adult Care: Wellness visits, chronic disease management, and urgent care.

Children’s Healthcare:

  • Wellness checkups, back-to-school and sports physicals.
  • Treatments for colds, flu, acne, weight management, and more.

Diagnostics: Blood work, EKG, urinalysis, COVID testing, pregnancy testing, and UTI testing.

Weight and Vitamin Management: Personalized consultations and care plans.

Home Health Services:
Home visits for patients in assisted living facilities or those unable to visit the clinic in person.

Telehealth Visits:
Remote appointments using secure telehealth software. Features include:

  • Privacy protection.
  • The option to record sessions.
  • Online scheduling for added convenience.

As part of the give-back program, Family First offers three free office visits each month to patients in need. To inquire about availability, simply give us a call.
