At Family First, our DPC membership includes:
- Up to 15 visits per year with rollovers for unused visits.
- Low-cost additional sick visits ($75 each after 15 visits).
- An annual wellness visit at no extra cost.
- Access to same-day and next-day appointments.
- Prescription writing, telehealth consultations, and ongoing support.

- $49 for individuals
- $149 for families
Monthly Membership Rates:
- Youth (12-18 years old): $69/month
- Adults (19-62 years old): $115/month
- Seniors (65+ years old): $125/month
